The Claim Form should be filled out online or submitted by mail if you are an individual who received notice of a data incident that occurred on or about March 31, 2022 and ended on or about April 27, 2022 (“Data Incident”) by letter from or on behalf of The Olympic Club (“Olympic Club”), and you wish to sign up for credit monitoring and identity protection services and/or had extraordinary losses because of the Data Incident or out‐of‐pocket expenses or lost time spent dealing with the Data Incident. You may get a check or electronic payment if you fill out this Claim Form, if the Settlement is finally approved, and if you are found to be eligible for a payment.
To start your online claim form, you can find your Claim Login ID & Claim Login PIN printed on the postcard Notice that was mailed to you. If you are unable to locate your ID & PIN, please contact us at (888) 836-1441 or by emailing info@OlympicClubDataSettlement.com. Please request your Claim Login ID & PIN by providing the Class Member’s full name and mailing address.
